I'm having several issues with the

 

Error 1 - This occurs every other time that I click the "Insert Template" button from the menu:

Errors 2 and 3 - these occur as the Make Template dialog is closing (either from the Save button or the red-X)

Click "No" to the "Do you wish to debug" prompt, and this follows:

Error: Object doesn't support this property or method.

The offending line of code here is, "top.close()"
