A while ago we upgraded to version 6.1.2 and have found that when inserting an image or media file that its placement is not where the cursor was.  For example we have a table with two columns in it.  We put the cursor into the right hand column, attempt to in an image and the image appears outside of the table.

 

Its causing us to tear our hair out and we need a solution ASAP.
