I had a similar problem with default configuration.

 

So I tried the simple to get all the icons out of the way.

 

Same problem.

 

I set the Autoconfigure to simple at the above example. It is identical to mine in icons except I have a second Paste Icon showing up beside the Paste Plain Text. This second paste icon, when clicked or mouse overed, functions as a delete (Which is

the next button in line) All remaining buttons are out of sequence. The first button on the second menu line in my application, is the help button. It is the last one on the previous line in the example.
