How do I remove the Insert Table icon from the toolbar?

    How do I remove the Insert Table icon from the toolbar?

    I tried adding InsertTable and Table to the DisableItemList but it does not remove the icon from the toolbar.  I am using the Default.config file which has these two lines pertaining to tables:
     
    <item type="table" />
    <!--     item type="image" name="InsertTable" imagename="instable" /-->
     
    What is the correct syntax to remove the Insert Table icon programmatically?
