Menu Images are not refreshing properly

The images on the menu are displaying correctly on ini page load, however clicking refresh will cause some of the images to not resolve, clicking refresh again will load some of the images (including some that where lost before) but others will be missing.

Please help

Are you using the version 4.0.0.5?
